How to use the STABLE Resource Toolkit

• Each section of the toolkit has a brief introduction to the clinical condition or issue that is covered and tools available in that section are listed.
• Every tool within a section is preceded by an overview page:
  • The purpose of the tool and its clinical utility is briefly explained
  • Scoring information is provided for tools that require numeric evaluation
  • Information regarding psychometric properties is provided, when needed, to guide use of the tool
• Each tool is available on a separate page:
  • The tools are formatted and provided without page numbers for office use
  • The tools can be reproduced for patient care purposes
